VERTIBRAL ARTERY TEST

WHY: Before performing neck work on clients who are elderly or have had major trauma, performing a vertebral artery test is a good idea. This test determines if the vertebral arteries that feed the brain are occluded or become impinged (shut off) when the client extends their head. If it is positive, you want to refer them to a physician.

HOW: Client lays supine. Therapist sits or stands at the head of the table, and hyperextends the client's head. Watch their eyes for 15 seconds and note if one or both pupils dilate. Ask them two simple questions (their name and where they are). Notice any signs of disorientation or confusion. If there are any of these signs, they may have an occluded vertebral artery. Do not perform any neck work & refer to a physician.

Most necks are 100% safe to work on, but because the neck is a particularly vulnerable area, our work must be precise, directed and gentle. Don’t just rub the neck muscles if you are not sure of the correct motions. Know what you are doing and why.
