In watching the video Lymphatic Drainage Massage it was not mention to work above the swelling/inflamation.
Is it okay to work on top of the swelling just along as we work toward the drainage site(s) of the area being worked

Yes, that is the point of lymphatic drainage- it is one of the only treatments you can/should do over an area of inflammation ie. a swollen sprained ankle, etc.
Good luck!

After a regular massage session I recomend my clients to take a special bath to help pull out the toxins that we have released (with special formulated products for that purpose) and the warmth of the water opens pores to help with removal.  Is a bath okay after a LMD session? I ask this in relation to your answer given about not applying detox oils with Swedish massage after LMD because that would increase blood flow, would the warm bath have the same effect?

Warm baths increase circulation, so I would not recommend it for people who have edema.
If the client does not have edema, and you are doing normal swedish massage, then a warm bath can be wonderful! I have personally experienced the powerful effects of epson salts in a bath, so I think that is a great thing!
